Westlock Trauma Form Description

The Westlock Trauma Form was created in 2013 in Westlock, Alberta, Canada by Dr. M. Firdaus M. Mydeen, Dr. L. M. Campbell, and A. Johnson. Westlock is just over 1 hour from the major trauma centres in Edmonton, Alberta. The Westlock Trauma Form is the brainchild of Dr. Mydeen, who saw the need for a standardized way of not only managing but also documenting trauma cases in the periphery.

The Westlock Trauma Form is a one-page resource that contains essential ATLS guidelines. It serves not only as a management guide but also as a platform for documentation. Checklists can be filled out and diagrams filled in. In this way, the Westlock Trauma Form facilitates the management of the trauma patient in the rural hospital but also can act as the patient's chart and can be sent with the patient to the city so that the receiving medical team can quickly and easily determine the patient's history and progress, as well as the interventions performed in the periphery.

The Westlock Trauma Form has been used in the Westlock Emergency room with positive feedback. Formal evaluation in a blinded trial during trauma simulations is planned for the summer of 2015.
